We conducted a self-management support program usinginformation and communication technology (the ICT program) for patients with cardiovascular disease. Our program consisted of educational program, homeexercise using internet and a device, and health educational counseling with nurses.We evaluated the physical and psychosocial effects of the ICT program. Monitoring through the ICT program was effective to improve patients motivation, QOL and psychosocial states. This program will be useful for patient with cardiovascular disease to continue their life style managements.

我们针对患有心血管疾病的患者进行了一种自我管理支持计划（ICT计划）。我们的计划包括使用Internet和设备的教育计划，HomeSercise以及与护士的健康教育咨询。我们评估了ICT计划的身体和社会心理效应。通过ICT计划进行监控可有效改善患者的动机，QOL和社会心理状态。该计划对于患有心血管疾病的患者可以继续其生活方式。
