This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. African-Americans are at high risk for hypertension and diabetes. Many of those treated do not have their condition brought under control according to standard treatment guidelines. Only 40% of African-Americans under treatment for hypertension are at goal for blood pressure (National Health and Nutrition Examination Survey). Only 12% of diabetes patients are at goal for blood glucose, blood pressure, and cholesterol (National Diabetes Information Clearinghouse). This study will examine the effectiveness of both physician education and patient counseling in improving the treatment outcomes of hypertension and diabetes patients in an Africa-American population. Increasing the percentage of African-American hypertension and diabetes patients at goal would reduce their risk of cardiovascular events and their disparities in health relative to the general population.
