基于“SHA2011”的老年人群慢性病医疗费用负担及控制策略研究

Aging of population and the burden of disease cost in the elderly population have become one of the most popular issues in the field of international health, and also has brought tremendous pressure on the development of health service. In order to understand the medical expenditure burden of chronic disease of the aged 65 and above and raise control strategies in our country, we will establish standardized database for medical expenditure in the aged by means of System of Health Accounts 2011（SHA2011）, which is the new internationally recognized health expenditure accounting system. Then we apply SPSS22.0, STATA12.0 statistical software to analyze the current situation and regional difference in the medical expenditure burden of elderly population, and horizontal, vertical and multi-dimension . Afterwards, we find out chronic diseases which spend more medical expenditure as a prior area for future disease control. We build up multiple regression model and grey correlation model to analyze the main factors of medical expenditure of chronic disease in the elderly population.Using STATA12.0 statistical software to account the burden of medical expenditure of chronic disease in the elderly population of each dimension under different financing arrangement. In the end, this research will raise control strategies about medical expenditure ,and related health insurance reform program about chronic disease in the elderly population. Providing intellectual support for the government makes health policy and builds a healthy aging society.

人口老龄化和老年人群疾病经济负担成为国际卫生领域最热门的议题之一，也为我国卫生事业发展带来巨大压力。为了解我国现阶段65岁及以上老年人群慢性病的医疗费用负担，并提出控制方案，我们基于“卫生费用核算体系2011“（SHA2011）这种新的国际公认的卫生费用核算体系，通过构建老年人群医疗费用规模标准化数据库，运用SPSS22.0、STATA12.0统计软件对老年人群慢性病医疗费用现状和地区差异进行分析，并进行横向、纵向和多维度比较研究，找出哪些慢性病消耗了更多的卫生费用，作为未来疾病控制的优先领域；建立多元回归模型、灰色关联模型分析老年人群慢性病医疗费用的主要影响因素；运用STATA12.0统计软件核算各维度不同筹资方案下老年人群慢性病的医疗费用。最后将提出老年人群慢性病医疗费用负担的控制策略，并给出配套医疗保险改革方案，为政府相关部门制定卫生政策、为构建健康的老龄化社会提供智力支持。

近年来，人口老龄化和老年人群疾病经济负担成为国际卫生领域最热门的议题之一。我国人口老龄化问题尤为严峻，为卫生事业发展带来巨大压力。我国因慢性病导致的疾病经济负担已占总疾病经济负担的70%，而65岁以上老年人慢性病患病率的增加，又通常是总人口的2-3倍。他们对卫生服务的需求不断增加，导致卫生总费用都在不断上升，这给政府、社会及家庭造成了严重的负担。研究老年人群卫生费用情况，对缓解老年人群医疗负担，改善其生活质量具有重要意义。. 课题组收集8省、1直辖市的卫生费用数据，建立标准化数据库并进行核算，按年龄、性别、疾病分类、参保类型、医疗机构类型、就医季节等多个维度划分出老年慢性病医疗费用的分布和构成，进行纵向分析；再根据地区发展状况，进行老年人群慢性病医疗费用的分布流向及地区间比较；最后对全国老年人群慢性病医疗费用规模、筹资公平性、影响因素等进行分析。研究发现，2016年全国老年人群经常性卫生费用为8157亿元，占总人群经常性卫生费用的30.2%。经常性治疗费用是7562亿元，主要发生在医院，占比77.23%，以住院费用为主（81.2%）。老年男性和女性的费用占比分别为52.66%和47.34%。在治疗费用中公共筹资占62.4%，家庭卫生支出占33.2%。主要引起老年人群疾病负担的是循环系统、呼吸系统、肿瘤、消化和内分泌、营养和代谢疾病，这5类疾病消耗了整个老年人治疗费用的70%。在住院总费用的构成中，占比较高的为药品费、化验费、检查费。. 本研究的科学意义在于：通过多维度阐明老年人群慢性病医疗费用的分布构成，厘清费用流向、筹资公平性问题，找出了影响医疗费用的主要因素。为政府相关部门制定政策提供科学依据，对指导医疗保障制度改革具有重要的应用价值。改善老年群体的健康状况是推进健康中国建设的重要步骤，关乎社会和谐安定。发展社会保障以缓解老年负担顺应的是民生诉求，解决的是民生疾苦，化解的是社会矛盾与经济危机，促进的是国家认同、社会公正与全面发展，维系的是社会安定与国家安全。
