中国西部地区城市新移民健康公平的社会决定因素与管理模式研究

With the fast development of urbanization in China, it is already a common phenomenon that the urban new immigrants actually are half-urbanization. Though the urban-rural health integration improves the balanced development of health service between urban and rural area, there are still some problems in the health equity of urban new immigrants. The most current studies on the urban new immigrants mainly focus on their survival situation and the lack of relevant rights from the perspective of sociology and economics. The health status of urban new immigrants and the way to include them into health integration development are seldom discussed. This project intends to anlayze the social determinants of health equity of urban new immigrants in Western China under the background of urban-rural health intergration using the social determinants of health theory. The project also aims to conduct the Fishbone analysis and structural equation modeling to explore the primary health influential factors, pathways, dynamic mechanism, and the key points. Based on the findings, the study intends to develop a management model that includes the urban new immigrants into the urban-rural health integration development, and conduct the empirical study on the model. The research results would contribute to improving of the primary public health services equity among all residents in the urbanization process and achieving health equity between urban and rural residents, and provide some references for similar research and policy making.

随着我国城市化进程的加快及城镇化建设的推进，城市新移民的"半城市化"状态已成为一种普遍的现象。在城乡卫生一体化逐渐促进城乡医疗卫生事业均衡发展的同时，城市新移民的健康公平性尚存在一些特殊的问题。现有对这一人群的研究大多从社会学或经济学角度，主要研究城市新移民的生存现状及相关权利缺失等问题，对这一特殊群体的健康问题及如何纳入卫生一体化发展的研究较少。本研究拟在城乡卫生一体化发展的背景下，运用健康社会决定因素理论对影响中国西部地区城市新移民健康公平的社会决定因素进行分析，采用"鱼骨图"法及结构方程模型等方法探讨主要健康影响因素、路径及动力机制，并找出关键节点，构建将城市新移民纳入城乡卫生一体化发展的管理模式并进行实证研究。研究结果对我国在"城镇化"推进过程中如何促进所有公民的基本公共卫生服务均等化，实现城乡居民的健康公平有重要的现实意义，并可为类似的研究及政策制定提供参考依据。

随着我国“城镇化”的推进，及城乡卫生一体化逐渐促进城乡医疗卫生事业的均衡发展，城市新移民的健康公平性存在的一些特殊问题有必要加以关注。本研究立足于中国西部，采用定性与定量相结合的方法，对西部地区城市新移民的健康状况及健康公平性进行评价，运用健康社会决定因素理论对影响城市新移民健康的社会决定因素及路径进行分析，并提出将城市新移民纳入城乡卫生一体化发展的管理模式。为在西部地区“城镇化”推进过程中促进所有公民享有基本卫生服务，实现健康公平提供参考依据。.总体来说，本研究已取得预期的研究成果。.一、揭示了中国西部地区城市新移民的基本特征、健康状况及健康公平性 城市新移民的文化程度偏低，56.2%的调查对象为小学及以下文化程度，大专及以上的仅占4.3%；收入也低于城市居民，新移民月收入在1500元以下的占40.5%。各类人群内部均存在健康的水平不公平；新移民的预防保健服务利用低于城市居民；38%的被调查新移民参加的是新农合，而参加新农合的被调查者92.2%的人医疗保健自付比例≧30%。研究结果为进一步完善新移民健康公平的相关政策提供了参考依据。.二、构建了评价中国西部城市新移民健康公平的社会决定因素分析框架，找出了健康的主要影响因素及关键节点 根据健康社会决定因素理论，结合城市新移民的特点构建了评价中国西部城市新移民健康公平的健康社会决定因素分析框架，并用该框架梳理影响城市新移民健康的社会决定因素。构建的健康社会决定因素分析框架为今后同类研究提供了借鉴，填补了该方面研究的空白。.三、提出了将城市新移民纳入城乡卫生一体化发展的管理模式 在对国家及各地新移民相关政策研究的基础上，结合本次定量及定性研究发现，构建了将城市新移民纳入城乡卫生一体化发展的管理模式，从制度机制建设、保障措施、公共服务利用、健康指标评价等方面进行了探讨。研究成果为在西部地区“城镇化”推进过程中促进所有公民享有基本卫生服务，实现健康公平提供了参考依据。
