基于时间、空间、情境的老年群体健康不平等的测度、演化及干预研究

The health inequality of the elderly not only hinders the realization of healthy aging, but also is not conducive to the harmonious development of the whole society. Existing studies have found that health inequality exists in the elderly in China. However, the evolutionary characteristics and mechanism of this phenomenon have not been analyzed due to the neglect of the timeliness of health acquisition and change..Focusing on the scientific issue of the evolutionary characteristics and mechanism of health inequality among the elderly in China, this project will use multi-disciplinary theories and methods such as health economics and sociology, and construct a life course research framework of three-dimensional mechanism of time, space and situation to study the following contents. Firstly, this project will use the health inequality index to measure the degree of health inequality among the elderly. Secondly, this project will analyze the evolutionary characteristics of the health inequality of the elderly from multiple dimensions. Thirdly, this project will use empirical methods to clarify the evolutionary mechanism of health inequality, i.e. to study the mechanism of the early "long arm" phenomenon on the health inequality of the elderly and the intervention mechanism of adult experience on health inequality; to study the mechanism of age and cohort effects on the evolution of health inequality among the elderly; to study the contribution of social policy, family support and individual efforts to the health inequality of the elderly. Lastly, this project will propose intervention strategies for the health inequality of the elderly in China. This project aims to enrich the related theory of health inequality and provide the basis for the government to formulate health insurance policies for the elderly, which has important theoretical significance and practical guiding significance.

老年群体健康不平等不仅有碍健康老龄化的实现，而且不利于社会的和谐发展。现有研究发现我国老年群体存在健康不平等现象，但由于忽视健康获得和变化的时间性，未能剖析该现象的演化特征及其机理。.本项目拟围绕我国老年群体健康不平等演化特征及其机理这一科学问题，综合健康经济学及社会学等多学科理论与方法，构建时间、空间和情境三维因素作用机制的生命历程研究框架。研究以下内容：采用健康不平等指数测度老年群体健康不平等程度。多维度刻画老年群体健康不平等演化特征。使用实证方法明晰健康不平等演化机理，即揭示早期“长手臂”现象对老年群体健康不平等影响机理及成年经历对其干预机制；年龄和世代效应对老年群体健康不平等时期演化的作用；社会政策、家庭支持和个人努力对老年群体健康不平等的贡献。提出我国老年群体健康不平等的干预策略。本项目以期丰富健康不平等相关理论，为政府制定老年健康保障政策提供依据，具有重要的理论和实践指导意义。

老年群体健康不平等不仅有碍健康老龄化的实现，而且不利于社会的和谐发展。现有研究发现我国老年群体存在健康不平等现象，但由于忽视健康获得和变化的时间性，未能剖析该现象的演化特征及其机理。本项目综合了健康经济学及社会学等多学科理论与方法，从生命历程视角研究了我国老年群体健康不平等问题。具体研究了老年群体健康不平等的程度及其演变趋势、形成原因及对策建议。研究发现我国存在与社会经济地位相关的老年健康不平等现象，且高龄老年群体的健康不平等程度较深。童年期和成年期家庭社会经济地位与老年健康不平等相关，家庭社会经济地位较高者，老年期健康水平较高。生活方式、医疗卫生服务可及性、营养获取、累积劣势／优势等机制可以解释社会经济地位对老年健康不平等的影响。本项目的研究成果丰富了健康不平等相关理论，为政府制定老年健康保障政策提供依据，具有重要的理论和实践指导意义。
