《中医七情量表》研制及其考评的不同定性方法的比较研究

‘less qualitative vs more quantitative approaches’, and ‘Less Chinese medical vs more Western medical diseases’ are the major problems in Chinese Medicine instrument researches. It is an urgent need to retain the traditional Chinese medicine cognitive and the subjective essence of scale evaluation. This project intends to develop the "Chinese Emotional Instrument" which could assess Chinese people’s emotions, and to clarify the key techniques and suitability of qualitative methods in the scale study..Abiding by the international research guidelines, following the different qualitative models, which including Grounded Theory, Interpretative Phenomenological Analysis, and Thematic Analysis performing the data collection and analysis through the various observation, interview, Delphi investigation and articles reviews, we first form and optimize the concept framework and items bank of the instrument, and compare the running ability among different qualitative methods with codes test information and subjective feelings, et al. Then, combined with cross-sectional and cohort design, we stratified random select patients with XI (Manic), NU (Mental and behavioral disorders), SI (Anxiety), BEI/YOU (Depression), JING/KONG (Frightened) emotions and healthy people to fill in the preliminary scale. The reliability, validity, item response theory model parameter, and interpretability etc. were analyzed, the Computerized Adaptive Testing and data simulation were used to develop the computer survey platform. At the same time, framework analysis and content analysis were combined to evaluate the scientific properties of different qualitative methods with credibility, extrapolation, and reliability, et al. Ultimately, we develop the "Chinese Emotional Instrument" and establish the hybrid methodological system, which including appropriate qualitative technologies, for Chinese Medical instruments researches.

中医量表研究方法学“轻定性–重定量”，病种“少中医–多西医”，如何回归量表主观测评本质、保留中医传统认知是亟需解决的问题。本项目拟明确定性方法在量表研究中的关键技术和适宜性，并研制《中医七情量表》，为国人的情绪测评提供科学工具。.遵循国际量表研究规范，首先采用扎根理论、解释现象学分析、主题分析的定性模式，通过观察、访谈、德尔菲调查和物化法进行收据收集分析，构建优化概念框架和条目库，并采用编码信息量、受试者主观感受等对比各方法的运行能力；再结合横断面和队列研究，分层随机抽样情绪为喜、怒、思、悲忧、惊恐的受试者和健康人填写量表初表，分析信/效度、条目反应理论模型参数、解释度等，并采用计算机自适应测试和数据模拟技术，研发量表的计算机测评平台；同时，结合框架分析和内容分析，评议不同定性模式方法的可信性、外推性、可靠性等科学属性，最终研制《中医七情量表》，建立包含适宜定性技术在内的量表混合方法体系。

中医量表研究方法学“轻定性–重定量”，病种“少中医–多西医”，如何回归量表主观测评本质、保留中医传统认知是亟需解决的问题。. 本项目遵循国际量表研究规范，通过文献研究、临床研究、访谈等多种方法进行数据收集，采用主题分析、扎根理论、现象学分析、内容分析、框架分析等多种定性方法技术，构建优化概念框架和条目库。明确了不同定性模式方法的在中医量表研究不同阶段的关键技术特征和适宜性，即主题分析适用于概念框架以及条目库构建的初始阶段，与扎根理论相比，框架分析更适用于概念框架的构建和优化；与现象学相比，内容分析更适用于条目库的构成；德尔菲法适用于量表条目的筛选。为同类研究提供了可借鉴的思路与方法，完善了中医量表研制的方法学体系。同时研制了包含悲忧、喜狂、愤怒、思虑、惊恐五个领域，43个条目的《综合情绪量表》初版纸质版量表。. 最后结合横断面和队列研究，分层随机抽样情绪为喜、怒、思、悲忧、惊恐的受试者和健康人填写纸质版初版量表，运用经典测量理论对条目进行定量筛选，继而采用项目反应理论对条目进行综合评价筛选以及条目参数评估。依据量表条目的能力参数值，采用计算机网络技术，建立量表工作场，明确测试的起始点、条目选择规则、计分运算规则和终止标准，采用蒙特卡洛模拟评估了其内部真实性，构建了《综合情绪量表》的计算机测评平台，为现代多种情绪的测评和管理提供了工具。
