The FiftyForward All of Us Program will address FiftyForward’s customer base and related individuals, including: o Members of our 7 FiftyForward lifelong learning centers o Customers of our Supportive Care Services programs, including: o Meals on Wheels o Adult Day Services o Victory Over Crime o Living at Home Care Management o Care Team and Conservatorship o FiftyForward volunteers o Foster Grandparent Program (FGP) o Retired Senior Volunteer Program (RSVP) o Friends Learning in Pairs (FLIP) o Children served by volunteer programs, and their family members o Family or friends of FiftyForward members or customers o Community members o Donors o Staff, Board, Advisory Board, and Committee Members o Health care professionals and other nonprofit organizations in partnership with FiftyForward  Geographically, our centers and programs serve individuals from the Middle TN area (including Bedford, Cheatham, Coffee, Davidson Dickson, Marshall, Maury, Robertson, Rutherford, Sumner, Trousdale, Warren, Williamson and Wilson Counties). We impact approximately 20,000 individuals each year.  Demographics vary by program, but eligibility for most FiftyForward Supportive Care Services programs includes being 50 years of age or older, needing assistance to access services, and having limited income and assets (specific income limits vary by program).  Some of our programs have additional eligibility requirements of having no family members available to assist in care, being the survivor of a crime, have physical or mental disability, or have difficulty safely preparing meals or lacking resources to obtain meals. Volunteer programs, such as the Foster Grandparent Program (FGP), serve a combination population: FGP engages adults age 55+ with incomes at or below 200% of federal poverty level, but also serves children from kindergarten to fourth grade living in Nashville’s seven school districts reported as having the highest incidence of violent crime, unemployment, percent of poverty, and largest number of households headed by a single female. 100% of the students served qualify for the Metro free lunch program, 97% are African American, 30% receive special education services, and 35% read at or below first grade level.  Our target population predominantly speaks English or Spanish. Through our centers, programs, and partnerships with approximately 50 other nonprofit agencies, we anticipate that we will be able to raise awareness for 5000 individuals through the distribution of written All of Us information; educate and engage 1000 individuals per year through information sessions, community activities, health fairs; and enroll a total of 110 volunteers over the 3-year program.
