The discharge nursing program developed through this research was consisted of five elements: approach for patients, approach for professional team members, approach for family/people who close to patients, approach for myself (nurse), and approach for organization/system. Through applying the program in clinical practice, it is hoped that community care for long term inpatients will be promoted.

通过这项研究制定的出院护理计划包括五个要素：患者的方法，专业团队成员的方法，接近患者的家庭/人的方法，对我自己的方法（护士）以及组织/系统的方法。通过在临床实践中应用该计划，希望能够促进长期住院医师的社区护理。
