T hisstatement oftheCommittee on Advanced Cardiac Imaging andTechnology ofthe Council on Clinical Cardiology addresses the use ofpositron emission tomography (PET)as a clinical toolforthediagnosis andmanagementof patients withcoronaryartery disease. Overthepast decade PETtechnology hasevolved tothepoint that clinical PET studies can beperformed safely and efficiently intheevaluation ofpatients withknownor suspected coronaryartery disease. Twospecific uses ofPET inmanagementofcoronaryartery disease havebeenproposed: 1)noninvasive detection of coronaryartery disease andestimation ofitsseverity and2)assessment ofmyocardial viability inpatients withcoronaryarterydisease andleftventricular dysfunction. Thediagnostic useofPETwouldapply toa large numberofpatients, whereas itsuse to assessmyocardial viability wouldapply toa smaller, more select numberofpatients. AsofFebruary 1991, theavailable datathathave emerged fromthefewcenters withsufficient experienceincardiac PETindicate thatthis promising and advanced imaging modality canoften provide accurate information inbothoftheseclinical situations. In addition, coincidence detection provides a means of measuring tissue photon attenuation forsubsequent correction ofemission data.
